Social Prescribing Link Workers

Life can be hard. You don't have to face it alone.
Sometimes things outside of your health can affect how you feel. This could be feeling lonely, worrying about money, struggling with housing, caring for someone, or feeling low and lacking confidence.
Our Social Prescribing Link Workers are here to listen and help. They will take the time to understand what matters to you and work with you to find the right support.
Listen. Connect. Thrive.
Social Prescribing is a free service for patients aged 18 and over.
Your Social Prescribing Link Worker can help you find local services, groups and activities that may improve your health and wellbeing. Together, you can look at your options, set goals and take small steps towards positive change.
